In order to report our census info, we must know your year of birth, not just the month and day. We will keep this confidential.

Other Family Members

Please list the names and birthdates of family members.

Children 6 years and up are eligible for membership.

If you wish to move your membership from another church, please enter the name of the church so we may contact them.

List your occupations above, so that we may create a searchable database for carpentry, plumbing, advertising, etc. Please specify which job is done by which person.

Church Directory

You will be added to the directory as a member, once you join the church.

Offering Envelopes

Would you like a box of offering envelopes? We don't require them, however if you donate with cash we need them so we can send you a statement at the end of the year.

New Member Info

If you are requesting membership registration, you will be contacted by the Pastor.

If you decide to come down and be publicly welcomed during the service, you will NOT need to fill out the paper membership form during or after the service.
